Bassett Healthcare Network (and its affiliates) is pleased to offer member information via a Web-based version of our computer systems called MyBassett Health Connection.

We think it is important for you to know how we handle information we communicate via the Internet. This Terms & Conditions statement outlines our practices and our sensitivity to your right to privacy.

Response to Electronic Communication: Bassett Healthcare Network (and its affiliates) will make its best effort to provide a timely response to electronic inquiries. In some cases, the clinic staff that needs to respond to an electronic inquiry or other communication may not be immediately available so a patient should allow three (3) business days for a response. Accordingly, emergency situations requiring immediate attention should not be submitted electronically.

Furthermore, with respect to any electronic communications sent by the patient, we are only able to respond to such communication based on the information provided by the patient. If there is insufficient information provided, we will be unable to provide accurate and reliable services.

Bassett Healthcare Network (and its affiliates) do not endorse and has not verified the accuracy of the information in/on these Websites, and the patient should not rely on any of the information found on the Website for purposes of treatment or diagnosis.

Minor Eligibility: An individual must be age 18 or older to request a MyBassett Health Connection account. Parents or guardians of minors (under the age of 18) may request access to the minor child's account by following the terms outlined in the "Proxy Access" section of this Terms and Conditions statement. Parents or Guardians of minor children between the ages of 11 and 17 (inclusive) will not be eligible to participate in the full spectrum of MyBassett Health Connection services. Thus, when a minor patient reaches age 11, restrictions will be placed on access to information in MyBassett Health Connection.

Proxy Access: MyBassett Health Connection account users (patients) age 18 or older who have medical decision-making capacity may request proxy access to his or her MyBassett Health Connection account be provided to another adult individual by completing the Proxy Access Consent Form and Authorization for the Release of Medical Information and submitting it to Bassett Healthcare Network (and its affiliates) . Only one person will be granted proxy access to a MyBassett Health Connection account of a patient with medical decision-making capacity. A MyBassett Health Connection account will be activated for both the proxy and the account holder.

Legally authorized decision-makers of adult patients who lack medical decision-making capacity may request proxy access to a patient's MyBassett Health Connection account by completing the Proxy Access Consent Form and Authorization for the Release of medical Information and submitting it to Bassett Healthcare Network or its affiliated private practices.

Email Privacy: Patients who are users of MyBassett Health Connection should be aware that they will be notified via email when there is new medical information to be viewed on MyBassett Health Connection. This means that any person with access to a patient's email will be able to see this notification. This could include the patient's spouse, employer or anyone else that can access a patient's email account. Although no private medical information will be sent, the notification that new medical information is available by accessing MyBassett Health Connection may be information that a patient would not want others to know. Thus, patients should take this into account when providing an email address.

Please know that if you send us an email communication, it may be shared with staff at Bassett Healthcare Network (and its affiliates) to assist the physician in providing the patient's medical care. A patient's confidential medical information on MyBassett Health Connection will be accessible only to appropriate clinical staff.

Security and Confidentiality: Bassett Healthcare Network (and its affiliates) is committed to protecting the security and confidentiality of information stored in MyBassett Health Connection. Firewalls, passwords, encryption and audit trails are further used to safeguard your information. We shall identify the records released and note the time and date of access each time a patient accesses MyBassett Health Connection.

All access to MyBassett Health Connection is accessible through a secured and encrypted communications channel to our web server. You can tell when you are securely connected to MyBassett Health Connection by looking at the web server address field. If the MyBassett Health Connection web server begins with https:// (instead of http://), this webpage comes from our secure server. This means your MyBassett Health Connection data cannot be read or deciphered by unauthorized individuals. Also, to the extent you or your proxy authorizes an approved web application to access your MyBassett Health information, those applications will access MyBassett Health with the use of a secure connection between the application and MyBassett Health Connection.

You can also tell whether you are truly connected to Bassett Healthcare Network (and its affiliates) by viewing our digital certificate (the lock at the end of the web server address field). The digital certificate validates the identity of our web server and ensures the protection and security of your MyBassett Health Connection information.

User names and passwords provide two layers of authentication and are stored in an encrypted database that is isolated from the Internet. As a MyBassett Health Connection user, your role in maintaining the security of your medical information is: 1) Changing your password on a regular basis; and 2) keeping your login ID and password confidential. You are solely responsible to protect the security of your login ID and password and you alone control who outside of Bassett Healthcare Network (and its affiliates) may access your MyBassett Health Connection account. A person who is authorized to access your MyBassett Health Connectioninformation as a proxy will also have the ability to authorize an approved web application to access your MyBassett Health Connection information.

3rd party applications: For your convenience, and at your discretion, Bassett Healthcare Network provides the ability for 3rd party applications to access and import your health information. If you choose to implement this feature, you must establish a MyBassett Health Connection patient portal account, as your user name and password for MyBassett Health Connection would be required by the third party application to access and import your health information. Bassett Healthcare Network reserves the right to disable any 3rd party application if needed.

Laboratory Values: Results available via MyBassett may be automatically released to you prior to being reviewed by your provider. If you have questions about any of your results please contact your provider.
